What can I expect when I come to Utah Weight Loss & Natural Hormone Clinic?

Because everyone's situation is different, no course of treatment will be exactly the same. However, certain things can be expected.

How do I arrange for a telephone visit?

If you are unable to come to the clinic for a follow-up visit, we will be happy to schedule a telephone consultation at your convenience.  Available times for these consultations are on Tuesday or Thursday from 8:00 AM to 5:00 PM, and MWF from 1:00 to 5:00 PM.

To arrange a telephone consultation, please call the clinic and schedule an appointment just as though you were coming in to the clinic.  You will be sent a questionnaire either through the mail or via RelayHealth which you will fill out and return before your consultation. Charges vary depending on the reason for the visit and the length of the call.

What is the procedure for out-of-state patients?

If you live outside of Utah, the procedure is as follows:

  1. Call the clinic at (801) 272-1246 to discuss your needs, provide initial information and set up a time for an initial telephone consultation.

  2. Download and print the new patient paperwork.  Fill it out completely, being sure that you indicate the severity of your symptoms by number and not just with a check mark.  Then send the completed paperwork by mail to:

        Holladay, UT 84117-5125

  1. As with an in-clinic visit, you may need a saliva test kit.  If this is the case, we will mail one to you. The test will be performed the first thing in the morning, and again in the evening before bed.  You will then send the test in a pre-paid mailer to ZRT Laboratories for analysis.  It will take approximately two weeks for these results to come back to the clinic. Once your results are available, we will determine the appropriate treatment..

  2. Any prescriptions required will be compounded by a local compounding pharmacy  in Salt Lake City who will mail them to you.  Follow-up consultations are handled in the same way as a telephone visit. The easiest way to handle administrative questions and details is via RelayHealth.

Is Kathryn a Primary Care Provider (PCP)?

While Kathryn has practiced as a Primary Care Provider in the past, she now limits her practice to bio-identical hormone restoration therapies for both men and women, nutritional health and weight release.

Do you bill insurance companies?

We do not accept direct reimbursement from insurance companies, and payment is due at the time of service based on a fee-for-service schedule.  Most insurance plans have flex-spend accounts or out-of-network benefits.  We provide our Utah patients with the information they need to submit claims directly to their insurance company.

Please note that since we do not bill insurance companies directly, we cannot be involved in any aspect of claim submission or insurance reimbursement.  Patients are responsible for all communications with their respective insurance companies, including pre-authorization questions.

What methods of payment do you accept?

We gladly accept Visa, Mastercard, personal checks or cash.  We no longer take American Express or Discover cards.
